Use the Evaluation, Daily, Re-Evaluation, and Discharge chart types when:
Use the Evaluation option for the initial visit.
you need to see your clients or patients multiple times to address a specific set of symptoms, and
each note does not need to be able to stand on its own.
Use the Daily option for visits that focus on treatment.
Use the Re-Evaluation option for visits that focus on assessing progress.
Use the Discharge option for the last visit.
Use the Comprehensive chart type when:
you need each note to be able to stand on its own, or
you expect to see your client or patient only once to address today's symptoms.
Use the Wellness chart type to document a healthy client or patient session, such as a client wanting a relaxation massage.
The Wellness notes format is NOT appropriate when:
the client or patient has health problems or symptoms and is trying to relieve them,
a doctor referred the client or patient, or
an insurance is involved.
