Client Information

All you need to include here is your client's name.

If you send your SOAP Notes to an insurance company, you will also include a claim number and a date of injury.

If the client was referred to you with diagnostic codes or if you are allowed to diagnose, you can include the diagnosis here as well. You can also include the name of the referring physician. This is a matter of preference.
