Objective - Modalities

In this part of the SOAP Notes you record the modalities used, the location the treatments were applied, and the length of time they were applied.

For example:

  •  Deep tissue massage, upper right shoulder, 15 minutes.

  •  Application of heat, upper right shoulder, 10 minutes
